Please include, but do not limit yourself to, the following information:
- Name, Age, Gender, Height and Weight.
- Although your fitness will be assessed, please include your exercise background. Be very specific. When was the last time you exercised consistently? Classify your fitness on a scale from 1 to 10 with 1 being very low fitness (can’t jog/run a mile) and 10 being an accomplished endurance athlete.
- Do you have any medical problems that may limit the type of training you can do? Musculoskeletal pain, recovering from a heart attack, high cholesterol or high blood pressure.
- Are you starting an exercise program mainly to lose weight?
- If you’re training for an endurance event send me the type, distance and date of the event.

If you own your own exercise equipment and live in the Albuquerque area I'll show you how to get the most out of it. We'll test your cardivascular fitness to determine a starting point, and to make sure you stick with it I'll provide you with a month's worth of exercise programs designed specifically for you.
